Parent/Guardian Consent and Waiver of Liability

Please read this document carefully before your child takes part. It affects your legal rights.

About the activity. The Craft Club runs supervised arts and crafts classes for children. Sessions may involve scissors, glue, paint, markers, clay, beads and other small components, and take place in shared studio and community spaces.

Assumption of risk. I understand that participation carries inherent risks, including minor cuts and scrapes, slips, trips and falls, allergic or skin reactions to art materials, staining or damage to clothing and personal property, and incidental contact with other participants. I accept these risks voluntarily on behalf of myself and my child.

Release. In consideration of my child being permitted to take part, I release The Craft Club, its owner, instructors, employees and volunteers, and the owner and operator of the venue, from all claims, demands and actions for loss, damage or injury arising from participation, except where caused by gross negligence or wilful misconduct.

Indemnity. I agree to indemnify and hold harmless the parties named above against any claim brought by me or on behalf of my child arising out of participation, including reasonable legal costs.

Medical. I confirm my child is well enough to take part, and I have disclosed below any allergies or medical conditions relevant to their participation. I authorise The Craft Club staff to arrange emergency medical care if it is needed, and I accept financial responsibility for any resulting costs.

Supervision. I understand that I remain responsible for my child outside of class time, and that The Craft Club is not responsible for a child before a session begins or after it ends.

Authority. I confirm that I am the parent or legal guardian of the child named below and have authority to agree to these terms on their behalf.

This agreement is governed by the laws of the Province of Ontario.

I have read and understood the terms above, and I agree to them on behalf of myself and my child. I understand that typing my name below has the same effect as my signature.

Optional — I consent to The Craft Club photographing or filming my child during the class, and using those images to promote The Craft Club on its website, social media and printed materials. Leaving this unticked does not affect my child's participation.
