Waiver, Assumption of Risk & Parent Release

IMPORTANT — PLEASE READ CAREFULLY

This Agreement (the "Agreement") is made by the undersigned Parent/Guardian in favour of The Craft Club (the "Craft Club"), together with its directors, officers, employees, contractors, instructors, volunteers and agents (collectively, the "Released Parties"), in connection with the Participant identified below.

1. Activities

The Participant will take part in structured arts-and-crafts and creative skill-development activities and related entry to, exit from and use of program premises.

2. Acknowledgement of Risks

The Parent/Guardian understands that reasonable care and supervision cannot eliminate every risk. Depending on the project and age group, risks may include:

• cuts, scrapes, punctures or pinches from scissors, craft tools, wood, wire or other materials;

• choking, ingestion or inhalation hazards associated with small parts or craft materials;

• skin, eye or respiratory irritation, staining, sensitivities or allergic reactions to paints, glues, fibres, clay, dough, latex or other materials;

• splinters, minor burns where age-appropriate heated tools are used under supervision, and contact with wet or slippery materials;

• slips, trips, falls, collisions or interactions with other participants, furniture or equipment;

• illness or exposure to communicable disease; and

• other risks reasonably incidental to the particular project, premises and age-appropriate activity. The Parent/Guardian accepts the ordinary and inherent risks of participation that remain after reasonable safety measures are taken and agrees to review disclosed project-specific risks and instructions.

3. Parent's Personal Release

To the fullest extent permitted by law, the Parent/Guardian releases the Released Parties from claims belonging personally to the Parent/Guardian arising from the Participant's voluntary participation, including derivative claims for loss or expense, except to the extent such claims cannot lawfully be released.

4. No Purported Waiver of Non-Waivable Rights of a Minor

Nothing in this Agreement is intended to waive, release or compromise any independent legal right or cause of action of the Participant that cannot lawfully be waived by a parent or guardian in advance. This clause is intended to preserve non-waivable rights while recording the Parent/Guardian's informed acknowledgement of the nature and risks of the Activities.

5. Parent Responsibilities

The Parent/Guardian will provide accurate health, allergy, accommodation and emergency information, ensure the Participant follows reasonable safety instructions, and promptly advise the Craft Club of any change relevant to safe participation.

6. Third-Party Claims & Indemnity

To the fullest extent permitted by law, the Parent/Guardian will indemnify the Released Parties against third-party claims, losses or reasonable legal costs arising directly from the Parent/Guardian's own negligent or wrongful act, material breach of this Agreement, or knowing provision of materially false information. This indemnity does not apply to the negligence, breach of law or willful misconduct of a Released Party, and is not intended to require the Parent/Guardian to indemnify the Released Parties for a claim belonging to the Participant that cannot lawfully be waived.

7. Emergency Response

If an emergency occurs and the Parent/Guardian cannot be reached promptly, the Parent/Guardian authorizes the Craft Club to contact emergency medical services and to provide responders with relevant emergency information. The Craft Club does not undertake to provide medical diagnosis or treatment beyond first aid or emergency assistance within the training and authority of its personnel.

8. Governing Law & Severability

This Agreement is governed by the laws of the Province of Ontario and the applicable laws of Canada. Any provision that is invalid or unenforceable will be severed or read down to the minimum extent necessary.

9. Acknowledgement

The Parent/Guardian confirms that they have read this document, understand that it affects their own legal rights, understand the disclosed risks, and sign voluntarily.
