AUTHORIZED PICKUP & CHILD RELEASE FORM

1. Authorized Pickup Persons

The Participant may be released only to a parent/legal guardian or an individual listed below, unless a written change is accepted by The Craft Club. Government-issued photo identification may be requested where identity is not known to staff.

2. Persons Not Authorized to Pick Up

List any person who must not pick up the Participant. Where a restriction arises from a custody/access order or other legal instrument, provide a current copy or sufficient lawful documentation. The Craft Club will not adjudicate family-law disputes.

3. Changes to Authorization

Only a person with legal authority for the Participant may request a change. Changes should be provided in writing through the method designated by The Craft Club. In urgent circumstances, staff may use reasonable verification procedures before accepting a change.

4. Release & Safety

The Craft Club will not knowingly release a child to an unauthorized person. If staff reasonably believe that immediate release would create a serious and imminent safety risk, they may contact another authorized adult or emergency services as reasonably necessary. This provision does not authorize staff to detain a person unlawfully.

5. Late or Failed Pickup

If no authorized person arrives by the required pickup time, staff will attempt to contact the Parent/Guardian and emergency contacts. A 10-minute grace period applies for unforeseen delay, after which the disclosed late fee may apply. If the child remains uncollected and reasonable contact attempts fail, or if the circumstances create a safety concern, The Craft Club may contact police, a children's aid society or other appropriate authorities as reasonably necessary. Repeated late pickup may result in suspension or termination of registration.

6. Sign-Out Record

The Craft Club may maintain attendance and release records identifying the date/time and person collecting the Participant for safety and administrative purposes.

PHOTOGRAPHY / VIDEO / MEDIA CONSENT

OPTIONAL — NOT A CONDITION OF PARTICIPATION

This optional consent is provided to The Craft Club (the "Craft Club"). Declining or withdrawing promotional media consent will not, by itself, affect the Participant's eligibility to participate.

2. Permitted Purpose

Where consent is given, media may be used for the selected purposes to document programs, communicate with participating families, and promote or provide information about The Craft Club. The Craft Club will not sell or license a child's identifiable image to an unrelated third party for that third party's independent advertising.

3. Online Platforms

The Parent/Guardian understands that content posted publicly online may be copied, shared, indexed, stored or processed by third-party platforms outside The Craft Club's direct control and may be accessible outside Canada, subject to the platform's terms and applicable law.

4. Withdrawal

Consent may be withdrawn prospectively by written notice to info@thecraftclub.ca. The Craft Club will stop new uses within a reasonable period after processing the request. Withdrawal cannot necessarily retrieve material already lawfully printed, distributed, reposted by others or otherwise placed beyond The Craft Club's reasonable control before withdrawal.

5. Privacy

Media and related personal information will be handled in accordance with The Craft Club's Privacy Policy/Privacy Notice. Questions may be directed to the Privacy Contact identified in that policy.

6. No Compensation

No compensation is payable for uses expressly authorized by this consent.
